Neuropathic Low Back Pain

Abstract
Traditionally, neuropathic low back pain has been correlated only with radiculopathy. The problem with this limited view is that it ignores aggravation of neural structures other then the nerve root through peripheral and central mechanisms. Neuropathic low back pain is examined from a structural standpoint, distinguishing processes that start from chronic inflammation and mechanical compromise and cross into the realm of neuropathy with primary neurogenic pathophysiology. Treatment should focus on treating the whole person, not just the pain problem. In general, combinations of pharmacologic and non-pharmacologic treatments including nerve blocks are very important. I hope that clinicians will become more aware that neuropathic pain often is present in low back pain and with appropriate assessment and treatment; early institution of appropriate treatment could prevent the devastation of severe persistent neuropathic pain.
